Forms:

To apply for an Ocean Broadband service, print and complete one of the Ocean Broadband Application Forms below:

• If you are a residential applicant living up to 100km from Perth CBD: Zone A Application Form
• If you are a residential applicant living 101km or more from Perth CBD: Zone B Application Form
• If you are a business: Business Application Form
  

By signing the application form, you indicate that you accept the terms of the following documents:

•  Standard Customer Relationship Agreement
• Acceptable Use Policy
  
• Privacy Policy

Once we have received your application a representative from our installation department will contact you to organise a signal test/installation date.  Generally there is a two week waiting period from the time our office receives the Ocean Broadband Application to the date of your signal test/installation.
